Does Saffron Extract Actually Improve Sleep Quality?

Saffron extract improved both objective and questionnaire measures of sleep in adults with mild to moderate sleep disorder associated with anxiety, this randomized double-blind controlled trial found. Sixty-six subjects were randomized to a placebo (maltodextrin) or a saffron extract at 15.5 mg per day for 6 weeks. This is the kind of trial I want to see before I take a botanical sleep claim seriously. Most of the earlier saffron sleep work rested on questionnaires alone, or tested an isolated purified molecule like crocetin rather than the whole extract, and isolated compounds rarely behave the same way as the unrefined extract at a comparable dose of active compound. Here the investigators added actigraphy, which measures the intensity, amount, and duration of physical movement, so at least one outcome does not depend on how a subject feels about their night.

The dose is the detail I keep coming back to: 15.5 mg per day, which is at the low end of what has been tested. Published saffron extract doses run from 22 mg to 300 mg per day, and the one trial reporting no effect used 22 and 28 mg for 4 weeks. A smaller daily amount producing measurable change over 6 weeks is worth noting, though a single trial does not settle a dose-response question.

I would temper expectations on effect size. The improvements the authors report are increased time in bed and better subjective scores, not a demonstration that saffron reconstructs sleep architecture. The population also matters: these were people with mild to moderate sleep disorder associated with anxiety, and saffron’s strongest prior evidence sits in depression and anxiety. Some of the sleep benefit may run through the anxiety. That is still a clinically useful result for the right patient, but it is a different claim from a direct hypnotic effect.

Benzodiazepines and benzodiazepine-receptor agonists work, but their use is limited by tolerance and by an elevated risk of dependency, morbidity, and mortality with long-term use. Why This Matters For Health And Performance

Sleep disturbance is common. Over a year, the prevalence of general sleep disturbance is estimated at approximately 85%, and primary insomnia is diagnosed in around 10%. Beyond disrupting daily activities, these disorders can induce fatigue, attention deficits, mood instability, anxiety, and depression.

Insomnia is associated with a reduction in hippocampal volume, in daytime cortical GABA levels, and in recruitment of the caudate nucleus. In brain regions including the hippocampus and prefrontal cortex, the usual decline in activity during the waking to sleep switch is inhibited. A general overactivity of the arousal, emotion regulating, and cognitive systems probably drives the pathophysiology of insomnia.

Effective drugs exist, including benzodiazepines and benzodiazepine-receptor agonists, but their use is restricted by tolerance and by an elevated risk of dependency, morbidity, and mortality with long-term use. That makes the search for safe and efficient compounds without adverse effects worth pursuing.
